This reservation form is a “first-come, first-serve” form to reserve a spot to pick one of Ava & Teddy’s puppies once they turn 6 weeks & 1 day old. Not everyone that fills out this form will be able to get a spot on the list, but rest assured you will get another chance to reserve a spot on another litter.
